New Patient Registration

registration4If you wish to register at Port Appin Surgery please complete a Registration Application Form obtainable from the reception and return it to the surgery.

It is helpful if you could bring along your previous medical card. Please keep medical cards in a safe place.

All new patients are asked to complete a health questionnaire and invited to come in for an examination, usually by one of the nursing staff.

  • We may refuse an application to join the list if you do not live within the practice area. If you are unsure please discuss this with one of the doctors.

If you leave the practice area, then please contact the local doctor in your new location to arrange registration with a new practice.

Patients who are temporarily in the area can be seen during any of the regular doctors' consultation times -  please give your details to the receptionist.
